Oklahoma Highway Patrol (OHP) is investigating a fatality collision that occurred on Feb. 8 at 1:51 a.m. on I-40 westbound, four miles west of Henryetta.
According to the accident report by OHP Trooper Brian Warren, 38-year-old Tom Nunn of Henryetta was driving a 2017 Nissan Sentra westbound on I-40 when he departed the roadway to the right for an unknown reason and struck a tree.
Nunn suffered massive injuries and died at the scene, according to the report.
Condition of the driver and the cause of the collision are both under investigation. Seatbelts were equipped but Nunn was not wearing one. Roadway conditions were reported as dry. Currently, funeral services are pending with Integrity Funeral Home of Henryetta.
